Jemma Gilbert is currently Director of Transformation at Transformation Partners in Health and Care (formerly Healthy London Partnership). With expertise in large scale change, she has directed regional transformation programmes for over 20 years, implementing London’s vision to be the healthiest global city. Jemma has a strong track record in harnessing collective action through city-wide partnerships, engaging across sectors, communities and promoting lived experience. She developed London’s first strategy for Primary Care Transformation and currently oversees a portfolio of programmes designed to tackle health inequalities. These range in scope from developing new personalised, integrated or digital services, to tackling societal stigma and enhancing VCSE capacity. In 2021, she received an OBE for services to healthcare in London during the pandemic, successfully leading efforts to protect Londoners experiencing homelessness.
